Requests are accepted on an ongoing basis. Applications for Healthy Community grants for $5,000 or less can be submitted any time during the year. Grants greater than $5,000 must be submitted by noon the first Friday in May. Innovation Grants are reviewed twice a year, and must be submitted by noon the second Friday in May and December.
Park Nicollet Foundation is the philanthropic arm of Park Nicollet Health Services and actively seeks partnerships in health education, intervention and preventive care.
The Foundation's first responsibility is to communities served by Park Nicollet. The Foundation's primary role in this capacity is to: (1) promote sensitive, caring services for all members of the community; (2) encourage the commitment of community and Park Nicollet resources to respond to these needs; (3) provide support for programs that enhance patient care and foster community access to health care; and (4) serve as an ambassador for Park Nicollet.
Grants are awarded in two categories:
(1) Healthy Community grants: This board-designated fund focuses on community organizations (including Park Nicollet Health Services programs and services that respond to community needs). Focus is on programs for families and children that improve health through documented urgent and compelling needs. Requests for funding through the Healthy Community Fund must document an urgent and compelling need, clearly offer or serve programs for families or children that improve health and build developmental assets. Grant proposals must focus on community needs within the Park Nicollet service area and meet two or more of the following focuses: (1) show evidence of collaboration with organizations that have similar missions for optimum community impact; (2) initiate or support health-related programs in response to expressed community needs; (3) provide support for prevention and education initiatives; (4) provide services to Park Nicollet patients and families; (5) partner with schools. (2) Innovation grants: Grants are awarded for demonstrated projects that support the health, healing and learning of those Park Nicollet serves. Grant applications must include a demonstrated partnership with or within Park Nicollet Health Services. The chief originator can be any nonprofit organization or component of a nonprofit organization. Any request that demonstrates a relationship with at least one Park Nicollet Health Services department will be considered.
