The annual grant cycle is as follows: Applications available: February 28; Application Final Due Date: May 31; EMSAC Evaluation Complete: June 30; Awards Notification: July 31; Fund Disbursement: September 30. Only one application per eligible, licensed EMS agency will be accepted. It is recommended those intending to apply schedule an appointment with the EMS Bureau Regional Consultant well in advance of the due date, May 31, to review the application and answer questions.
This fund was established to assist EMS agencies in acquiring EMS vehicles and equipment for use by EMS personnel in the performance of their duties. Applicants must include with applications:
(1) Completed Request for Taxpayer Identification Number and Certification (W-9);
(2) County or city endorsement: A minimum of one letter of endorsement from a county or city government within the EMS agency's primary response area, in Idaho, must be submitted with the application to achieve grant eligibility.
(3) Price quotes for requested vehicle(s);
(4) If vehicle is being replaced, include two (2) pictures, front and back.
(5) Include a copy of the registration or title of the vehicle being replaced.
(6) Each requested vehicle(s) requires a separate narrative page titled for that item and priority number.
(7) Equipment price quote(s) for each equipment item being requested.
(8) Each requested equipment item requires a separate narrative page titled for that item and priority.
Requirements for vehicles to be purchased: Vehicle must be for sole purpose of providing emergency medical services. All requested vehicles must meet current industry standards for the intended purpose of that vehicle. No vehicles designed for the mounting or carrying of fire fighting equipment or supplies will be funded. Requested vehicle(s) must be appropriate for agency level of licensure. Ambulance vehicle funding will only be awarded to licensed ambulance agencies. Non-transport vehicles may include medical rescue or rescue extrication capabilities. Snowmobiles, boats, ATVs, trailers, fire fighting equipment, etc., are not eligible for grant program.
Other equipment requirements: Disposable equipment is not eligible, except for Epinephrine auto injectors. Requested equipment must be appropriate for agency level of licensure. If more than one item is requested, each item must be listed on a separate line in priority order, except for items that come as a set.
